Telecommunication Annual Questionaire System
The Telecom Annual Report password should be used for this application. If you have questions about the password, please contact the utility employee responsible for the Telecom Annual Report.
Telecommunications Provider Annual Questionnaire Change Password
NOTE: The password can only be changed by someone with access to the primary email address for the carrier
3 password change attempts are allowed per day, if all attempts are failures, then the account will be dissallowed from changing it's password for 24 hours.
A confirmation email will be sent to the provider’s official contact email currently on file with the Commission. You must be able to access that email account in order to change the password.
Passswords must contain at least:
- 12 characters, but can not exceed 32 characters
- 1 uppercase letter (A-Z)
- 1 lowercase letter (a-z)
- 2 special characters from the following list: !#$%()@?_ (includes spaces, but leading and trailing spaces will be removed.)
- 2 digits (0-9)
Instructions:
- Choose your utility name from the dropdown menu.
- Check the "I'm not a robot“ box.
- Click the Send Password Reset Code button. This will send a passcode to your company’s primary contact email. That email is shown in the box.
- Enter the code from the email in the one time code box.
- Enter your new password.
- Enter your new password again in the confirmation text box.
- Click the save new password button.
